該表其實分為兩個部分:

第一部分：失分率統計

該部分的主要任務在於分析小題的失分率和整張試卷的失分率。一般來說一張試捲成績的好壞與小題的准確率關聯很大。所以我們要倍加重視小題。在每次考試之後，都要進行統計，同學們會發現，每次小考的小題的平均失分率基本上就是你大考的失分率。

第二部分：錯因分析

該部分的主要任務是針對整張試卷的所有錯題的錯因進行統計。這類的錯因有兩種。

第一種是知識性錯誤：此錯誤為單純的沒有學會該部分知識導致的錯誤。此類錯誤是易於修改的，只需要考後對該部分知識點進行一定的強化訓練即可。

第二種是習慣性錯誤：此類錯誤成因很多，也是同學們最頭疼的錯誤，頭痛的原因在於即使犯了這種錯誤，也知道自己有這種錯誤，但是就是不知道如何才能修改這類錯誤。其實究其原因就在於同學們沒有真正的去挖掘錯誤發生的真正原因，基本上一筆帶過。

例如高中階段經常聽到同學們提到的馬虎，不認真。

這樣的回答其實都是籠統的。同學們在整理錯因的時候，不要這么去寫，要進一步分析原因，然後寫上去。下面我針對馬虎這類錯誤進行一個示範分析：

1、急於去做題，導致讀題速度過快 ，而忽略了其中某些關鍵詞

2、在寫答案的過程中，急於完成書寫過程，所以寫第三行的時候想著第四行怎麼寫，

導致筆誤產生的錯誤

3、由於時間不夠，導致想盡快做完題目，所以沒有進行任何分析，就寫答案了。

4、在進行開方的時候沒有寫出兩個解。

諸如以上的馬虎成因的分析，才算是真正的抓住問題的本質，然後進行統計。這樣才能在以後做題中進行有針對性的修正。同時要統計每類錯誤所犯的總次數，例如：

1、此次考試這類錯誤所犯的次數--可由家長代為統計

2、往次考試這類錯誤所犯的次數--可由家長代為統計

這樣同學們經過多張試卷的分析後，就會對自身所犯的錯誤次數列出一個排行榜，榜單錯誤次數前三名的錯誤，就是同學們著重要分析解決的問題，同時也是下一次考試檢查的重點環節。從而提高學習效率和考試效率。

3、大考成績綜合統計表--可由家長代為完成

該表的一些選項可能在某些考試無法統計，比如說市排名，區排名甚至排名，那麼有哪個就寫哪個。關鍵在於前面成績的統計。
第一個用處：能夠較為客觀的分析同學們階段性的學習狀態，為下一步制定學習計劃

提供依據

第二個用處：用於單科分析。

例如數學，我們可以通過幾次考試的數學成績的波動情況，分析各階段同學們的學習狀態，找到學習該門課程的低谷期和學習難點，進而可以有目的的制定復習計劃
